Story: a 7 month old infant with developmental delay

Lonwabo has been referred to the early intervention clinic because at 8 months he was not  using his one hand as much as the other and his sitting balance was poor

When I first meet Lonwabo, he was sitting on his mom's lap and was alert and looking around, but when I pulled up a chair so that I could sit facing him and his mom, Lonwabo started to cry.

So I moved my chair back, sat down, greeted them both and chatted to his mom Kuhle about her concerns for her baby. Kuhle had also noticed that  he was not using his left hand much and that the left arm felt stiff when she was dressing him. 

We also chatted about her family and living arrangements.  Kuhle has two older children aged 4 and 8. Kuhle's mother and sister live with them and help to take care of the children when Kuhle is at work during the day.  Getting to the clinic is difficult and regular visits for early intervention will not be possible. 

While chatting mom Kuhle I observe Lonwabo's behaviour. He is sitting with his back to his mom and she supports him around the waist.  His open right hand rests on his mother's forearm arm. His left arm is flexed close to his chest and the hand is fisted. 

Next I start to paly with an easy to hold rattle, passing it from one hand to the other  and gently shaking it from time to time.  Lonwabo watches this game, looking to his mom from time to time and also looking up at me and then back at the toy. I give the rattle to his mom so that she can pass it to him. He reaches out with his right hand, takes the rattle, looks at it carefully and then shakes it a few times. 

I pick up another rattle and hold it within easy reaching distance. He drops the one in his hand and takes the new one again with the right hand.
